The Adult and Child Medical History forms are electronic documents so you will be able to fill them out on your computer and then print them to bring with you to your appointment with Dr. Keller. Once you have closed your browser, all the information on the form will be deleted. They may also be printed as blank forms and filled in by hand and brought to your appointment.
